Important Information About Child Health Plan Plus (CHP+)
CHP+ in Colorado will end on January 31, 2018, unless Congress passes a law to renew its federal funding.

  • As of today, there have been no changes to your CHP+ benefits. Please use your CHP+ benefits as usual.
  • Your best source of information during this time is CO.gov/HCPF/Future-Child-Health-Plan-Plus-CHP. Check this site frequently and click on CHP+ Member & Applicant Resources. The state will update this page right away if Congress takes action to renew CHP+ funding.
  • Create an account and sign up to get information from the state by email or text at CO.gov/PEAK.
  • Log on to your CO.gov/PEAK account and make sure your information is up to date.
  • If you would like to keep your provider if CHP+ ends, ask your provider’s office for the names of the private insurance plans they accept. Write this information down in case you need to shop for a private insurance plan. You can buy a dental plan throughout the year on connectforhealthco.com.

Please note that our CHP+ network is changing, effective June 1, 2017. Please be sure to check the CHP+ Provider Directory to ensure that your provider is still part of the CHP+ network. If they are not listed in the directory, you can find another dentist in the directory or contact customer service at customer_service@ddpco.com or 1-800-610-0201.

Child Health Plan Plus (CHP+) is a program designed by the State of Colorado. It is intended to provide medical and dental coverage for uninsured Colorado children through age 18. The program provides assistance to families that earn too much to qualify for Medicaid but cannot afford private insurance.

The eligibility of participants in this program is determined by the State of Colorado. If a child is eligible for Medicaid, he or she is not eligible for CHP+.

Delta Dental of Colorado administers the CHP+ dental benefits portion of this program. CHP+ shares the commitment with Delta Dental of Colorado to improve the oral health of Colorado children by improving access to dental care.

A summary of dental benefits provided to CHP+ members include:

  • Diagnostic services: Annual exam and X-rays
  • Preventive services: Annual cleaning, fluoride, and sealants
  • Basic restorative services: Fillings and stainless-steel crowns
  • Oral surgery services: Some extractions
  • Endodontic Services: Root canals

Links to CHP + benefit booklets and provider directory are below. Double check the group number that your child is enrolled in and select the corresponding booklet. The group number is located on your child’s ID card. It is also important to check the effective date shown on the ID card that indicates the date your child can begin to receive dental benefits.
