At Delta Dental, we strive to improve the oral health of Coloradans. That's why we offer a variety of dental benefit plan options - including the Delta Dental Patient Direct plan. This product is NOT insurance. It's a discount dental program that uses exclusive dental networks. For a low annual cost, members have access to discounted services when receiving treatment from a Delta Dental Patient Direct dentist.

There are two separate Delta Dental Patient Direct discount dental programs available:

Delta Dental Patient Direct, a Discount Plan for Groups, is a uniquely designed plan that allows groups to offer a discount plan to its employees.

Members can save a significant percentage on certain dental procedures from participating dentists in this separate Delta Dental Patient Direct network of over 550 dentists. The Delta Dental Patient Direct Plan is a discount plan, not an insurance product. It's affordable, valuable, and easy to use.

• No waiting periods
• No annual deductibles
• No claims to file

Upon enrollment through an employer group, the member will need to list his/her selected dentist on the enrollment form. All family members are required to select the same general dentist. Delta Dental Patient Direct dentists will receive a list of enrollees each month; members receive an identification card. Once enrolled, that's it! Members can schedule an appointment with their selected dentist and begin to receive discounts.

Delta Dental Patient Direct, a Discount Plan for Individuals, gives you the benefits you need for the price you want. For an annual membership fee:

• You will have access to discounted services provided by participating dentists.
• You pay the dentist directly, eliminating the need to file claims or pay deductibles.
• You need only verify your dentist participates with the patient direct network.

Delta Dental Patient Direct^® is affordable. The annual membership fee is:

• $99 for individuals.
• $120 for families (an unlimited number of dependents under age 19 included).
