Child Health Plan Plus (CHP+) is a program designed by the State of Colorado to provide medical and dental coverage for uninsured Colorado children through age 18 whose families earn too much to qualify for Medicaid but cannot afford private insurance.
The eligibility of participants in this program is determined by the State of Colorado. If a child is eligible for Medicaid, they are not eligible for CHP+.
Delta Dental of Colorado administers the CHP+ dental benefits portion of this program.
CHP+ shares the commitment with Delta Dental of Colorado to improve the oral health of Colorado children by improving access to dental care.

Children may receive up to $600 of covered dental benefits in each calendar year
The dental program has a specific list of covered services.
The dental program has a specific list of network dentists that must be used.
Some procedures may require a copayment.
The EyeMed discount vision plan is included with the dental program.
If your child is currently enrolled in the CHP+ Dental Program, select from the following links for a Benefit Booklet or a List of Participating Dentists. It is very important that you select the correct group number that your child is enrolled in. The group number is located on your childs member identification card.

Important Information
Benefits for the CHP+ Dental Program are available only when services are provided by a dentist listed in the Delta Dental Participating directory. This is the directory that is given to you when your child is enrolled. If your child is treated by a dentist who is not listed in the directory that Delta Dental provided to you, NO benefits will be paid by Delta Dental and you will be responsible for the entire fee charged by the dentist.
