If the completed application, completed enrollment forms and binder check are received before the 20th of the month, the group will be eligible the first of the next month. The group will be sent a welcome letter once the application and enrollment forms have been processed. 

*Enrollment forms must be completed for all eligible employees enrolling in the plan at initial time of enrollment.

For small groups with 1 – 4 enrolled employees, you may download the Benefit Booklet for the plan design that is purchased.
